Many people do not think about what it takes to live well with a disability or age-related concerns, until they need help or a crisis occurs. Planning is the key to prepare for the many transitions that occur in one's lifetime
If you have questions about long-term care planning or questions about a current need, Golden Crescent Aging and Disability Resource Center can help you. We provide you with information to be able to make informed decisions regarding long-term care for you or your loved one.
GCADRC helps to connect older adults, veterans, and people with disabilities of all ages to long-term support services. We can assist in making referrals to community resources that can help meet the needs of individuals and enhance community living, personal choice and independence.
The overall goal of the program is to empower individuals by informing them of options available in their communities so they are able to make informed decisions regarding their health and other long-term support options.
The goal of GCADRC is to:
1) Serve as a visible and trusted source of information on the full range of long-term care options; by providing personalized and objective assistance by empowering people to make decisions about their support options.
2) Provide coordinated and streamlined access to long-term care programs through a simplified process.
3) Help people transition from hospitals and other institutional settings into the community. GCADRC serves as single point of entry into the long-term care service system and can help alleviate the frustrations many consumers and their families experience when they need to obtain information and access to supports and services.
